Section 7 and 8 Re-certification Form 2024-2025

Section 7:

If a student athlete participates in a Fall Sport during the 2024-2025 season. The student athlete will need to turn in a Section 7 Re-Certification for during Winter and Spring sports. 

If the Student athlete does not participate in a Fall sport, but participates in a Winter and Spring sport. They will need the Re-certification for the Spring sports season. 

This form is for Multiple Sport Athletes only. 

 

Section 8:

This form is to be completed by a Doctor or Physician following a Sports related injury and needs to be turn into the Athletic Trainer (Ang) before the Student Athlete may return to full participation.
